Cash, certified bank Checks or money orders, Credit/Debit cards are accepted at the counter. Certified bank checks or money orders are accepted through the mail.

You may order a certified copy of a birth or death certificate record online using your credit card through our only approved Internet ordering service, VitalChek Network Inc using the link below.

You may also apply for a birth or death certificate, by filling out the forms available on the Greene County Public Health webpage and mail it with money order, or use our drop box, NO CASH OR CHECK.

Greene County Public Health does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network Inc. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard®, or Visa®.

Other internet vendors are not authorized by Greene County Public Health and may charge additional fees without providing state-approved services. Neither the Greene County Combined Health District nor VitalChek have control over the services and fees other Internet vendors offer and charge, nor can we guarantee the confidentiality of any vital record data provided to vendors other than VitalChek

Birth and death records are public information and available to anyone upon request.

Description:

The Vital Statistics Office receives, records, and maintains records of all births and deaths occurring in Greene County from 1909 to present. All documents recorded prior to 1909 are located at the Greene County Records Center and Archives. Birth and death certificate information regarding the number of births, birth defects, and leading causes of death is maintained by the Ohio Department of Health.

Uncertified records are no longer available due to the passage of Ohio House Bill 95. We are sorry for any inconvenience that this may cause.
