To obtain a copy of an immunization record from Greene County Public Health

Please complete the Authorization to Release Immunization Record form and submit it with a copy of a Current Driver’s License, State Identification Card, or U.S. Passport

You may return the form by mail or email.

Please allow a minimum of 7-10 business days for copies to be prepared.

It is possible that if Greene County Public Health did not administer all of the vaccines you have received, your record may not be complete.
Ohio medical providers are not required to report immunizations to the State Immunization Registry.
Please get in touch with your healthcare provider who provided your vaccination.
